Practice Tookit: Resources for Assessing Practice Needs

Assessing specific practice needs is an important first step in identifying which local community partners will provide the strongest opportunity for collaboration. The Assessment of Chronic Illness Care (ACIC 3.5) was developed as a practical quality-improvement tool to help organizations evaluate the strengths and weaknesses of their delivery of care for chronic illness in six areas: community linkages, self-management support, decision support, delivery system design, information systems, and organization of care. More information about the development of the ACIC can be found on the Improving Chronic Illness Care website (www.improvingchroniccare.org).

ACCTION Pack allows you to complete a slightly modified, 34-item version of the ACIC to help you evaluate the existing resources for chronic care and prevention in your practice setting. You do not have to complete all 34-items at once - the system will save your questionnaires so that you can return to complete them later. You can also print the questionnaire, if you'd like to complete it on paper and return to enter your responses at a later time.

After completing the modified ACIC, ACCTION Pack completes a summary of your responses for your records. This short summary report includes recommended links to help you choose resources for improving your practice. From the summary page, you can also perform one or more "quick searches" of the ACCTION Pack Tool Selector to locate other resources within the database that can help you improve one of the six domains of the Chronic Care Model.

The modified ACIC is condition-specific. You should complete a separate questionnaire for each target condition or risk factor that you are trying to improve / address. You can also complete the questionnaire again in the future to determine if new policies or programs are making an impact on Chronic Care resources in your practice.
