The ACCTION Pack Toolkit has been developed by the AHRQ Practice Based Research Network-Resource Center (PBRN-RC) through a cooperative
healthcare initiative between the
Agency for Healthcare Research and Quality (AHRQ) and the
U.S. Centers for Disease Control & Prevention (CDC).
The result of this joint effort is a web portal to a wealth of information about guiding principles, case studies, and prior research for practical,
collaborative approaches to prevent and manage major chronic illnesses."

- a searchable toolkit of resources to help improve these services
- general guidelines for implementation strategies
There are two distinct sections of this site. One is geared toward helping a community professional to assess the community's needs while offering guidance on developing productive
partnerships with healthcare practices.
A parallel section is geared toward healthcare professionals and helps assess their preventive and chronic care strategies, while offering guidance on partnering with a community organization.
Both sections help a user to ultimately search a wealth of "tools" that include evidence-based assessment and intervention strategies, which are indexed according to the 5A's of behavioral counseling and the Chronic Care Model.
